  • NOTICE TO APPLICANT:

  • Do not sign this agreement before you have read it or if it contains any blank spaces. This agreement is a legally binding instrument. Both sides of the contract is binding only when the agreement is accepted, signed, and dated by the authorized official of the school or the admissions officer at the school’s principal place of business. Read before signing.

    You are entitled to an exact copy of this agreement and any disclosure pages you sign. This agreement and the school catalog constitute the entire agreement between the student and the academy.


    Although Michael Vincent Academy, Inc. will provide placement assistance, the academy does not guarantee job placement to graduates upon program completion or upon graduation. Michael Vincent Academy, Inc reserves the right to reschedule the program start date when the number of students scheduled is too small. Michael Vincent Academy, Inc. Reserves the right to terminate a students’ training for unsatisfactory progress, nonpayment of tuition or failure to abide established standards of conduct. Michael Vincent Academy, Inc also reserves the right to place a delinquent student into a faulty status, meaning that if a student is on a payment plan and has not made a payment for two weeks or longer, they Academy has the right to withdraw the student from the current courses they are attending and the delinquent student CAN NOT return to class, until they are at a positive status with their student account.


    Michael Vincent Academy, Inc. does not guarantee the transferability of credits to a college, university or institution. Any decision on the comparability, appropriateness and applicability of credit and whether they should be accepted is the decision of the receiving institution.

    It is a promissory note without the signed written consent of the student or his/her parent or guardian if he/she is a minor and a written statement notifying all parties that the cancellation and refund policy continues to apply unfair business practice for the academy to sell, discount, or otherwise transfer this contract or Any questions a student may have regarding this enrollment agreement that have not been satisfactorily answered by the institution may be directed to:

  • TERMS AND CONDITIONS MVA STUDENT PRIVACY POLICY

  • We are regulated by Department of Consumer Affairs Bureau for Private Postsecondary Education with Exempt Status pursuant to the California Private Post Secondary Education Act or 2009 (Act) Status. Education Code section 94874 states in relevant part: The following are exempt from this chapter: (d) An institution offering either of the following: (2) Continuing education or license examination preparation, if the institution or the program is approved, certified, or sponsored by any of the following:

    A) A government agency, other than the bureau, that licenses persons in a particular profession, occupation, trade, or career field.

    (f) An institution that does not award degrees and that solely provides educational programs for total charges of two thousand five hundred dollars ($2,500) or less when no part of the total charges is paid from state or federal student financial aid programs. The bureau may adjust this cost threshold based upon the California Consumer Price Index and post notification of the adjusted cost threshold on its Internet Web site, as the bureau determines, through the promulgation of regulations, that the adjustment is consistent with the intent of this chapter. Our team has exceeded program licensing requirements and are trained in all aspects of hair, skin, makeup and nail aesthetics, body care and related programs.

    ACADEMY CLOSURE POLICY

    If the Institute closes permanently and ceases to offer instruction after students have enrolled, and instruction has begun, the student shall be entitled to a prorated refund of tuition.

  • MVA COPYRIGHT LAW

  • All Michael Vincent Academy students, instructors and staff must comply with copyright laws and the provisions of licensing agreements that apply to software. Unauthorized distribution of copyrighted material, including unauthorized peer- to-peer file sharing, may subject the student and/or staff member to civil and criminal liabilities. Michael Vincent Academy will cooperate fully with any investigation by public authorities related to the unauthorized distribution of copyrighted material. Students, faculty and/or staff found guilty will be subject to the full extent of fines and penalties imposed, as well as facing disciplinary action from the Institute. Copyright infringement is the act of exercising, without permission or legal authority, one or more of the exclusive rights granted to the copyright owner under section 106 of the Copyright Act (Title 17 of the United States Code). These rights include the right to reproduce or distribute a copyrighted work. In the file- sharing context, downloading or uploading substantial parts of a copyrighted work without authority constitutes an infringement.

    Penalties for copyright infringement include civil and criminal penalties. In general, anyone found liable for civil copyright infringement may be ordered to pay either actual damages or “statutory” damages affixed at not less than $750 and not more than $30,000 per work infringed. For “willful” infringement, a court may award up to $150,000 per work infringed. A court can, in its discretion, also assess costs and attorneys’ fees. For details, see Title 17, United States Code, Sections 504, 505. Willful copyright infringement can also result in criminal penalties, including imprisonment of up to five years and fines of up to $250,000 per offense.   • CODE OF CONDUCT FOR ALL EDUCATIONAL RESOURCES:

  • Michael Vincent Academy strives to educate, inspire, and provide financial resources to all students so that they may achieve their goals and walk with confidence in this dream world of fashion, beauty and business! In compliance with the federal law, MVA Trainers/employees, and agents shall maintain exemplary standards of professional conduct in all aspects of carrying out his or her responsibilities, including all dealings with any guests involved in our business in any manner as well as students. Help students seek, obtain, and make the best use of all financial resources available and provide services that do not discriminate on the basis of race, religion, color, financial status, sex, ethnic origin, age, veteran status or sexual orientation. Respect and protect the confidentiality the students records and of the economic circumstances of the student and student’s family. Information will be released only on the written consent of the student and/or student’s family, and all policies and procedures shall protect the student’s right of privacy. Commit to the highest level of ethical behavior and refrain from conflict of interest or the perception thereof.
  • SAY NO TO DRUGS

  • Michael Vincent Academy believes that it is very important to provide a safe environment for all of its students and Team Members. Substance abuse, while at school seriously endangers the safety of students and Team Members. It creates a variety of problems including increased theft, decreased morale, decreased productivity, and a decline in the quality of services provided. It is also our Policy to prevent the use and/or presence of these substances in the school. Michael Vincent Academy will strive to provide an environment conducive to making conscientious and healthy decisions when students are faced with difficult choices associated with the use of legal and illegal drugs. It is a violation of school policy for any student to: Use illegal drugs or misuse prescription drugs; Misuse alcohol; Possess, trade, manufacture, distribute, dispense, buy or offer for sale alcohol, illegal and/or prescription drugs while on duty, during break periods, or on school property; Arrive or return to school intoxicated from use of illegal drugs, misused prescription drugs or alcohol; Engage in the use of illegal drugs, misuse of prescription drugs or alcohol during school hours or while on school property (including parking lots); Use prescription drugs or nonprescription drugs that may affect the safety of the student or fellow students, and members of the public. Any student, who is taking any prescription or non-prescription drug, which might impair safety, performance, or any motor functions, must advise his/her Instructor or an Institute Director before providing services under such medication. Students are individually responsible for being aware of applicable laws, regulations, ordinances, and the Academies policy for complying with them. Michael Vincent Academy will assist in that endeavor by providing current information on an ongoing basis to all students.
  • MVA COLLECTION POLICY

  • If the student is terminated or withdraws from our Academy, we shall inform the student of any balance owed to Michael Vincent Academy. The Academy is committed to using ethical business practices in connection with any collection efforts. Collection correspondence shall reference the Academy's cancellation and refund policies set forth in this policy. In the event that the student’s enrollment agreement is sold or discounted to a third party, such third party shall be required to comply with the cancellation and refund policies set forth in this policy. Since defaulting on a loan can lead to several legal consequences including possible exposure to a civil lawsuit. You may wish to consult with a lawyer if you are unsure of the status of your debt situation. Also, it may be very helpful to hire an attorney if you will be contesting or appealing any collection efforts initiated against you. Your lawyer will be able to present all your options to you and guide you through the appeal process. If for any reason your account becomes past due in which a payment has not been processed over 90 days late, it becomes eligible to be sent to a collections agency. The balance owed in addition to late fees and collection/legal fees will be processed into collections and sent to A collections agency.
  • CANCELLATION REFUND POLICY

  • REJECTION: An applicant rejected by the school is entitled to a refund of all monies paid to the academy with the exception of $500.00 NON-REFUNDABLE deposit.
    The Federal Trade Commission (FTC) has put in place several safeguards to protect consumers from making hasty contract decisions. In particular situations, there is a three-day cooling-off period during which a consumer can cancel a contract. Regardless of the location, a buyer cannot cancel a contract under the cooling-off period for a transaction that:
    • Totals less than $25;
    • Involves business or education goods (i.e., not personal or household goods);
    • Involves cars (even if it's at a car show);
    • Involves arts and crafts at a fair-type venue; or
    Should the student not receive his/her education goods the applicant who provides written notice of cancellation within the 3 day cooling off period, excluding weekends and holidays, of executing the enrollment agreement is entitled to a refund of all monies paid. Excluding classes already attended, non-refundable registration fee as well as courses enrolled with afuture start date or not yet taken. Michael Vincent Academy, Inc will not allow any returns or refunds for tools and kits. The following conditions must be met in order for the student to receive a refund less the registration fee and MVA beauty kit: A written notice must be mailed or e-mailed within the 3 day cooling off period to Michael Vincent Academy, Inc. Or A written notice delivered in person to Michael Vincent Academy.

  • CANCELLATION NOTICE TO ACADEMY

  • If your contract is eligible for the three-day cooling-off period- you need to give cancellation notice to the Academy.

    Make sure your notice includes:
    • Your name, address, and contact information;
    • Identifying information for the goods or services you're wishing to cancel (e.g., order number, account number, etc.); and
    • An unequivocal statement that you are canceling the contract.

    As long as you send the notice by mail,— or even better, hand delivery -- within three business days of the sale, the company may not sue you for breach of contract.

    MICHAEL VINCENT ACADEMYS ACCEPTED REASONS TO CANCEL:

    • If MVA is permanently closed and/or no longer offering instruction of the course after the student has enrolled, the student shall be entitled to appropriate refund of tuition as described in this contract.

    • If a student suffers a catastrophic accident, a death in the family, or circumstances beyond the ordinary and reasonable control of the student, the academy will make a reasonable and fair settlement in the best interest of both par ties, i.e., MVA and student/family of student.

    • If the student does not notify the academy that the student is withdrawing, formal termination shall be based on monitoring of participation determined by the academy.

    • Students may be terminated for violations of academy rules, poor attendance, improper conduct, or failure to maintain satisfactory progress as outlined in the agreement. No additional fees are applied for such causes of termination; the refund calculation for monies is as outlined below.

    ORIENTATION • On the 1st day of class Student is required to come 30 minutes early prior to starting his/her first class.

    MODELS

    • All models must be over the age of 18.
    • All models are required to sign a release of liability and ph oto release.
    • If a Model is a student, student shall receive a $50.00 MVA Bucks credit towards their account for their time. • Beauty Artistry: Two models are required (1) male model for drag makeup class. (1) Female model for the final class.
    • Barbering Artistry: (15) Male models are required. (12) Hair models for class and (3) hair models for the final. Must be present 2 hours after class begins for a regular class. For final must be present with you on time. Females are allowed but must have hair that students can practice on.
    • Hair Artistry: One model is required. Preferably with mid back or longer hair.
    • Airbrush Artistry: No models are required for this course.

    WITHDRAWAL PROCEDURES:

    If balances are owed on the date of termination or withdraw, the student is responsible for the payment in full of the balance owed on that date. If student fails to make arrangements to pay off the remanding debt, student or parents/legal guardian will be sent to collections.
    1. A student choosing to withdraw from MVA after the start of classes is to provide a written notice to the director of the school. The notice must include the expected end date of attendance and be signed and dated by the student.
    2. If special circumstances arise, a student may request, in writing, a leave of absence. This must include the date in which the student anticipates the leave beginning and ending. The withdrawal will occur on the day the student is scheduled to return from the leave of absence, but fails to do so.
    3. A student will be determined to be withdrawn from the institution if the student misses four (3) instructional days.
  • REQUEST TO TRANSFER ENROLLMENT TO A DIFFERENT CLASS DATE

  • MVA will allow students to transfer enrollment to a future certificate course or a different day as long as the following conditions are met: A “Request to Transfer Enrollment” form is emailed by student to info@mva.la two weeks notice prior to cancellation date.
  • ADDITIONAL FUTURE CLASSES

  • In the event, a student takes additional courses here at Michael Vincent Academy, all term and conditions are valid for any class the student decides to add. No additional contract is required to be signed. Student will be financially responsible for any additional he or she wants to add.
  • YOUR DEPOSIT

  • If You have 60 days to get started with a class. Your $500.00 NON-REFUNDABLE dollar deposit will expire, no refunds will be given and student must provide an additional deposit should they want to attend a class. There are no refunds on the $500.00 deposit. The student can avoid expiration by doing a minimum payment of $250.00/month.

    ORIENTATION

    ATTENDANCE
    • Zero tolerance on absences.
    • Student is required to make up the class prior to receiving a Certification.
    • If you are on a payment plan with MVA, payment is still require.
    • Any absences will result into a $25.00 fee.

    TARDINESS
    • A tardy is if you are 30 minutes late. • Your instructor will not allow you in class.

    CLEAN-UP
    • All students are required to clean up their work stations.

    HOMEWORK
    • Homework is required of all students. If student fails to turn in homework for a class, the student will be asked to re-take the class.
    • Homework must be turned into your instructors company e-mail or tag instruct on Instagram.

    KIT
    • If kit needs to be shipped, the student is required to pay all shipping cost.
    • All kits will be released 5 business days after paying tuition in full.
    • Beauty Artistry: If you are not paid in full, on the 1st day of class, you will receive your brushes, palette and spatula. When you are paid in full you will receive the rest of your kit.
    • Barbering, Airbrush, Hair Design Artistry will receive your kits on the first day of class.
    • Back-ordered Kits: You will be notified and given updates on the status of our kit.

    INTERNSHIP
    • The consistency of Internships vary and are never guaranteed by Michael Vincent Academy.
    • Additional Rules and Regulation apply please read “Internship Guidelines” below
    • Profanity is never allowed while on an internship. If profani ty is used will report immediately.

    PAYMENTS
    • If you pay more towards your tuition, your weekly payment is still required.
    • Class is Canceled-Payment is required
    • Holiday- Payment is required
    • Absent- Payment is required
    • Weekend Classes- Payment is required on Friday or Monday
    • If you miss a payment you are still responsible for that payment and will need to make this payment along with the next scheduled payment
    • $500 Deposit is NON-REFUNDABLE
    • If you are delinquent on your auto-pay account for two or more weeks Michael Vincent Academy, INC has the right to pull the student from their current course(s) until they are no longer delinquent on their student account (subject to missed payment fees).

    MISSED PAYMENT
    • %10 late fee of the payment amount due, on late payments.

    UNIFORM
    • Must be worn at all times while on the MVA campus.
    • Must be worn while in class and while on internship

    REFERRAL FEE
    • If you refer anyone to us you will receive a $50.00 dollars worth of MVA dollars. It allows you to purchase event tickets, product, or put it towards your tuition.
    • Student must enrolled

    KITS/TOOLS
    • Upgrades: Are allowed on the first day of class. All items must be sealed and unused.
    • Refunds: No refunds on kits that have already been disburse.
    • Stolen: MVA does not take responsibility for any lost, stolen or damaged items.
    • Makeup Brushes: If brushes have loose bristles, MVA will replace the brush once within 30 days of receiving brushes.

    PARKING/PERSONAL PROPERTY
    • Parking is free for all students.
    • Park at your own risk: Michael Vincent Academy is not responsible lost, stolen, or damage property
    • You are parking at your own risk.
    • If Parking is full and student must pay for parking, Michael Vincent Academy, Inc will not validate or reimburse any parking fee’s
    • Michael Vincent Academy is not responsible for any lost, stole or damage property.

    LOST AND FOUND
    Michael Vincent Academy’s basic policy for handling privately owned property that was lost in our facility and found by staff members, students, and/or visitors.
    • All staff, student and visitors are required to turn in unatte nded and or lost items.
    • Safeguarding lost property: All personal property will be kept in MVA locked room.
    • Returning lost property to the rightful owner: If you claim lost property you will be required to provided basic contact information.
    • Disposition of unclaimed property: MVA will dispose of the unclaimed property with in 60 days.
    • Record keeping and reporting: MVA will keep a record or the date, location and a brief description of the item that was lost.
    • Michael Vincent Academy, Inc is not responsible for any lost, stolen, or damage property.

    RETAKING A CLASS
    • There is a $499.00 fee for a re-takes per course.
    • Should the student fail any exams the student will be required to pay the re-take course fee.
    • Effective August 1, 2015. All student with an enrollment date of August 1, 2015 will no longer be eligible for “unlimited education”

  • GRADUATION REQUIREMENTS IN ALL COURSES

  • • Receive the required number of training hours. You cannot miss more that ten (10) clock hours of classes at MVA in our 6-week pro beauty course and our 12-week pro beauty course.
    • Complete and receive passing grades in all projects
    • Pass all written and practical (hands on) exams
    • Must pass final exam with 75% or better
    • Receive MVA graduation certificate

    GOLD SEAL
    If you pass both the written and hands on exam you will receive the “Gold Seal”. If you receive a gold seal on your certificate, this means you passed your test and MVA is able to Certify you. Your certificate will read “CERTIFIED”.

    RETAKE EXAMS

    I have read and I understand all of Michael Vincent Academy Rules and Regulation. I understand and agree to consequences some of these rule have. My signatures is confirmation and agreement of these Rules and Regulation.

I HEREBY VOLUNTARILY ASSUME ALL OF THE RISKS OF PARTICIPATING AND/OR VOLUNTEERING FOR THIS CLASS, ACTIVITY OR EVENT.

    I certify that I am physically fit and have not been advised to not participate by qualified medical professional. I certify that there are no health-related reasons or problems, which preclude my participation in this activity or event.

    I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said class, activity or event. In permitting me to participate in this event, I hereby take action for executors, my administrators, heirs, next of kin, successors, assigns, and myself as follows:

    (A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event. THE FOLLOWING ENTITIES OR PERSONS: MICHAEL VINCENT ACADEMY and/or their directors, officers, employees, volunteers, managers, representatives, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers; (B)I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or person mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this event to the fullest extent permitted by law.

    I acknowledge that MICHAEL VINCENT ACADEMY/and their directors, staff, officers, volunteers, managers, representatives, and agents are NOT responsible for The errors, omissions, acts, or failures to act of any party or entity conducting a specific event or activity on behalf of MICHAEL VINCENT ACADEMY.

    I acknowledge that this activity or event may involve a test of a person’s physical and mental limits and may carry with it the potential for injury, stress and property loss. The risks may include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, actions of other people including, but not limited to, participants, volunteers, spectators, instructors, and event monitors, and/or producers of the vent. These risks are not only inherent to participants, but are also present for volunteers.

    I understand that at this event or related activities, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the event holders, producers, sponsors, organizers, and assigns.   • I have carefully read and received an exact copy of this enrollment agreement.

    I understand that MVA may terminate my enrollment if I fail to comply with attendance, academic, and financial requirements or if I fail to abide by established standards of conduct, as outlined in this agreement. While enrolled in the school, I understand that I must maintain satisfactory academic progress as described in school catalog and that my financial obligation to the school must be paid in full before a certificate may be awarded.

    I understand that the academy does not guarantee job placement to graduates upon program completion, or upon graduation understand that any cancellation and/or withdrawal from Michael Vincent Academy must be submitted in writing within three (3) days of commencement.

    I understand that I am unable to request a refund for all kits and told received. I understand that I am unable to request a refund for any educational goods already received and completed. I have read and understood the FTC 3 day cooling off period.

    I understand that the $500.00 deposit is NON-REFUNDABLE.

    I understand the nature of this paperless contract. I am aware of the primary purpose in collecting personal information is to provide you with a safe, secure online application which will assist me in sending and signing my contracts.   • AFFILIATE PROGRAM TERMS AND CONDITIONS

  • PURPOSE

    • By definition the Affiliate Programs enables Michael Vincent Academy, to bring you into close association or connection with the Industry. If the connection benefited to enhance your level of professionalism then the MVA Affiliate Industry Program has served its purpose. The Affiliate Industry Program is a proven way to gain relevant knowledge, skills, and experience.
    • You're not just perfecting a talent, you're starting your career. Receive a unique opportunity to potentially have your work published and receive credit in major magazines. This platform is for our most skilled students helping them bui ld their resumes to provide a higher opportunity in landing a j obs in the field. Whether you want to take your career to Hollywood or be a freelancer the MVA Affiliate Industry Program is for you.
    • Work side by side with A-D list celebrities, celebrity make up artists, film directors, producers, publicists, photographers and videographers who have excelled in fashion shows, on set productions, weddings, magazine pictorials, retail and much more.
    • A-list- A term that alludes to major movie stars, or the most bankable in the Hollywood film industry or to major recording artists, major international sports stars or miscellaneous occupations such as the most successful film directors, certain high profile media and entertainment moguls and the most notable international TV broadcasters.
    • B-list – These people usually work frequently and are often well known by the public, but they don’t command the highest salaries or attract the biggest audiences. Popular television actors can fall into this category, and there are many well-known character actors and actresses that seem to be always working.
    • C-list- People on this list are usually "famous for being famous" or reality show stars. To qualify to be called a "C-lister" you must be blissfully unaware that you aren't as famous as you claim to be or have received more fame than is proportional to your skills and talent.
    • D-list- A D-List celebrity is simply a person who is known simply through social networks, a person who has made a name for them selves through social networking sites such as Youtube, Facebook, Instagram, Twitter or what ever else is out there. D-List celebrities are not necessarily known for there talents but work put in, areas such as video production, photography, game play or even just simple creativity and originality.

    ACADEMIC REQUIREMENTS:
    IN ODER TO PARTICIPATE IN THE MVA AFFILIATE INDUSTRY PROGRAM YO U MUST BE A CERTIFIED MAKEUP ARTIST, CERTIFIED HAIR DESIGN ARTI ST, LICENSED/CERTIFIED BARBER BY THE MICHAEL VINCENT ACADEMY. THE COURSE MUST BE COMPLETED WITH A PASSING GRADE. FEE FOR AFFILIATE INDUSTRY PROGRAM MUST BE 80% PAID.

    TIER 1
    • Have passed both written, and hands on exams with a 75% or higher
    • Must have all homework turned in.
    • Approved by instructor.
    • You will be considered main artist and will have an opportunity to have an assistant artist.
    • Affiliate Tuition must be 80% paid off

    TIER 2
    • Has paid for tuition but did not pass the written, or hands on exam. (One or the other must have passed at least one exam and is scheduled to re-take the exams)
    • Has not been approved by instructor to be a tier 1.
    • Will be allow to go to internships as an assistant.

    ROLES AND RESPONSIBILITIES
    The affiliate role and responsibilities are to at all times represent the MVA company to their best ability and to make sure those all around and involved know we are professionals. We have many relationships within the industry and would love to keep them for future collaborations.

    The affiliate program provides exposure and access to an industry that is not usually available to individual who has less than 2 years experience. The student is expected to function as in a professional manner, which is reflected in the projects and activities performed and in relationships with the organization's administrators and other staff.

    Affiliates are expected to:
    • Complete their given task.
    • Provide professional quality work.
    • Use their own Makeup, hair kit, barbering kit, and airbrush k it.
    • Comply with the policies and procedures of the organization.
    • Integrate within the structure of the organization.
    • Take initiative in designing or implementing a project.
    • Expected to be attentive to his or her models on a regular basis.
    • Complete academic and professional assignments related to the placement.
    • Receive a signature on his/her proof of attendance from the producer, on site manager, lead makeup artist.

    TEAM LEADERS/ON SITE COORDINATOR ROLE AND RESPONSIBILITIES

    Each student is assigned a Team Leader or on Site Coordinator to help with any questions. The team leader is there to insure the students are comfortable and have everything that they need. The Field Supervisor visits the site to assess student progress and discuss whether or not there has been problems or if the customer has had their expectations met.

    NOTICE: You will not be paid for Internships. These opportunities are solely for the purpose of exposure, experience, and portfolio building. MVA is not opposed to affiliate receiving tips or donations.

    • One day equals to one Affiliate Opportunity: Example: if a shoot runs for 3 days, MVA has complied to 3 of those opportunities.
    • If you cancel a schedule on set opportunity, it will count as one.
    • Quality of opportunities may never be determined by the affiliate. MVA works hard creating connections for students to gain exposure and experience.

    PARKING/PERSONAL PROPERTY
    • Park at your own risk: Michael Vincent Academy is not responsible lost, stolen, or damage property while on an affiliate opportunity.
    • You are parking at your own risk. Michael Vincent Academy Is not responsible for any lost, stole or damage property while parking
    • Michael Vincent Academy is not responsible for any stolen, lost, damage kits while on Internships

    RELEASE OF LIABILITY
    I WAIVE, RELEASE AND DISCHARGE FROM ANY AND ALL LIABILITY, INCL UDING BUT NOT LIMITED TO: LIABILITY ARISING FROM THE NEGLIGENCE OR FAULT OF THE ENTITIES OR PERSONS RELEASED FROM MY DEATH, DISABILITY, PERSONAL INJURY, PROPERTY D AMAGE, PROPERTY THEFT OR ACTION OF ANY KIND WHICH MAY HEREAFTER OCCUR TO ME INCLUDING MY TRAVEL TO AND FROM ANY EVENTS. THE FOLLOWING ENTITIES OR PERSON S, MICHAEL VINCENT ACADEMY/MICHAEL VINCENT PHOTOGRAPHY/ IMAGE G ROUP PR AND OR THEIR DIRECTORS, OFFICERS, EMPLOYEES, VOLUNTEERS, MANAGERS, REPRESENT ATIVE AND AGENTS, THE ACTIVITY OR EVENT HOLDERS, THE ACTIVITY A ND EVENT SPONSORS, ACTIVITY OR EVENT VOLUNTEERS. I INDEMNIFY HOLD HARMLESS AND PROMISE NOT TO SUE THE ENTITIES OR PERSON MENTIONED IN THIS PARAGRAPH, FROM AN Y AND ALL LIABILITIES OR CLAIMS MADE AS A RESULT OF PARTICIPATION IN THIS EVENT TO THE FULLEST EXTENT PERMITTED MY LAW.

    I. RULES AND REGULATIONS
    DRESS CODE
    __Professional: Black Attire
    __Dress to Impress: Something comfortable
    __ Michael Vincent Academy shirt is to be worn at all times.
    __ Club attire is not permitted: Over exposure clothing is not permitted
    __Make sure all clothes are cleaned and pressed
    __No open toed shoes, no flip flops
    __High heals higher than 2 inches are not permitted
    __No see through clothing. (IE: Legging, yoga pants, shirts, shorts, skirts.)
    __ Basic hygiene is mandatory
    __Grooming is mandatory
    __ Alway carry mints, body spray and deodorant.
    __ Not baseball hats
    __ No baggy clothing, No Jerseys, No bananas
    __ Nails must be clean and manicured.

    ARRIVAL ON SET
    __Check in with Team leader
    __You are to check in with MVA team leader or on site coordinator from MVA.
    __If team leader has not yet arrived, gather as a team, stay ou t of the way and wait until they get there.
    __Please arrive to the set 15-20 minutes early
    __Parking: Bring cash. Most Affiliate Industry Program are Los Angeles which require meter or parking payments. Michael Vincent Academy will NOT reimbursement parking. BEHAVIOR/CONDUCT ON SET
    __Do not solicit: Do not give out business cards while on set. After the job has been completed, please ask MVA team leader for permission to hand out your business card.
    __If you are asked for a business card or any other
    information, redirect them to the MVA team leader. __ Safety: Do not give out any personal info. Must Always carry business cards or MVA business Cards.
    __Do not take pictures unless authorized by production company or site manager.
    __Do not ask the talent for autographs, gifts, or favors of any kind.
    __Under any circumstances will you never be start struck by the talent. You will carry yourself in a professional manner.
    __If pictures are allowed, make sure you ask your MVA team leader when would be the best day to release them on social media.
    __If you show up drunk, high, or appear to be on any illegal substance you will be asked to leave the premises immediately. A report will be submitted to Michael Vincent Academy, Inc Administrative Depart and you will be placed on a 2 week suspension from our internship program .
    __Never accept, alcoholic beverages, or illegal substances from anyone while on set.
    __Profanity: Profanity is never allowed while on set
    __Phones: No personal call while on set. No personal Texting while on set. No personal e-mail while on set.
    __Food: Never eat the food that is placed for the talent, company or crew unless offered to have some by the MVA team leader or on site manager.
    __No friends or family allowed on set.
    __No children allowed
    __No pets allowed
    __Do not wander off while on the set. Stay in assigned area unti l you have relieved of your duties.
    __ If offered a cash tip, you may accept it. Under no circumstances will you place a tip jar requesting money. Unless approved by MVA team leader.

    CONSISTENCY OF INTERNSHIP
    __ Michael Vincent Academy will provide a minimum of five (5) Affiliate Opportunities are provided with a $2,500.00 fee. If affiliate chooses to have more opportunities He/she must pay for additional opportunities.
    __ Michael Vincent Academy is not required to provided advance notification.
    __ On set opportunities may vary from: Music videos, fashion shows, Expo’s, commercials, up and coming talent, A-list celebrities, photo shoots, Magazine test runs, Magazine shoots, Marketing Campaign Ads and many more.

    NO CALLS/NO SHOWS
    __Please contact/Inform ahead 24 hrs if you are not going to be able to be there the day of there event.
    __Expect to be on set for full production. You may be asked to say longer than expected.
    __ A full work day is considered to be a 8-10 hour shift SANITATION
    __When reporting to an affiliate assignment you must arrive with sanitized kit. MVA will not be held responsible should you cause a client to have a reaction due to unsanitary tools and/or makeup, or hair tools.
    __MVA advises you to sanitize, clean and wipe all tools and products in front of your client to make sure they know you are hygienic and follow health and safety precautions and procedures. Also, use a metal spatula to scoop or scrape products from original containers and place them on a palette to work on instead of dipping directly into creams, liquids, or gel formula products. Expect to be on set for full production. You may be asked to say longer than expected.

    DEFAMATION
    Defamation, which consists of both libel and slander, is defined by case law and statute in California. See Cal. Civ. Code §§ 44, 45a, and 46.
    • The elements of a defamation claim are: publication of a statement of fact that is false, unprivileged, has a natural tendency to injure or which causes "special damage," and the defendant's fault in publishing the statement amounted to at least negligence.
    • Publication, which may be written or oral, means communication to a third person who understands the defamatory meaning of the statement and its application to the person to whom reference is made. Publication need not be to the “public” at large; communication to a single individual other than the plaintiff is sufficient. Republishing a defamatory statement made by another is generally not protected.

    I acknowledge I have read and understood the contents of this form, and have been given full opportunity to discuss the implications of this consent of my own free will and my decision is not based upon representations or advice by representatives of the Academy.
